To save these template files on your hard drive, click on the link for the form. Netscape will prompt you that this is an unknown file type. Click the Save File button.

You will need to save this template file in the the TEMPLATES subdirectory. The TEMPLATES subdirectory will either be under the MSOFFICE directory, or under the WORD or WINWORD directory, depending on what version you are using. Use the browse button to locate the correct subdirectory. In MS Office 97, the TEMPLATES directory is located under C:\Program Files\Microsoft Office.

To use the template files, follow these steps:

1. Download the file(s).
2. Open Word.
3. Select File - New
4. Select the ITC_Form template to start a new document. Please note that when you open the template, it may prompt you for a password. Ignore the password, and click on the Read Only button.
5. Fill out the form, using the TAB key to move from field to field.
6. Print the form.
