Required Forms

The forms available here are needed by you, and your camper to register for TTU camps. Please sent the filled out forms and mail or fax to to the TTU Cheer and Dance camp. DO NOT SEND THESE COMPLETED FORMS TO UCA OR UDA.

Medical Release Form: This form is for parents of campers. This is a required form for camp registration.

Adult Release and Waiver (Advisors/Coaches/Chaperones): This form is for any of the Advisors accompanying the campers. This form is required for a camp registration.

Participant Release Form: This is another form for campers. This form is also required for camp registration.
