Medical Release Form : This form is for parents of campers. This is a required form for camp registration.
Adult Release and Waiver (Advisors/Coaches/Chaperones): This form is for any of the Advisors accompanying the campers. This form is required for a camp registration.
Participant Release Form : This is another form for campers. This form is also required for camp registration.
