MMR (Measles, Mumps, and Rubella)--Proof of Two Doses
Proof of the FIRST DOSE must be submitted prior to registration in the first semester for FULL TIME graduate students (9 hours or more).The SECOND DOSE proof must be provided prior to registration a second semester
Exceptions:
- Students born prior to January 1, 1957
- Students who provide a
Vaccination Religious Objection Statement stating that their religious tenets and practices prohibit their receipt of any type of immunization - Students who provide written documentation from a physician certifying they are allergic to the MMR vaccine
- Students who provide written documentation from a physician attesting they have a diagnosed case of measles, mumps, or rubella
- Students who provide written documentation from a physician of their laboratory confirmed immunity to measles, mumps, or rubella
- Students who graduated from a public or private high school in Tennessee from May 1999 to date
- Students who attended a public or private school in Tennessee for grades K-12 for any period of time on or after July 1, 2001
- Students who graduated from a public or private Tennessee high school between May 1979 and December 1998 are not required to provide proof of immunization with the first dose of the MMR vaccination. However, students who graduated from a public or private Tennessee high school between May 1979 and December 1998 are required to provide proof of receipt of the second dose of the MMR vaccination
- Students who are classified as active duty military personnel and provide proof of their active duty status
- Students who were previously enrolled as a full-time student at a TBR college or university for at least one full semester after August 1, 2007
Meningococcal ("Meningitis") & Hepatitis B
All students enrolled in one or more courses must submit the signed Meningococcal Meningitis and Hepatitis B Immunization Health History Form.
TB Skin Test
All international students beginning Fall 2009 must provide documentation of the TB Skin Test.
