Which form should I fill out if....

• I need to update my organization's roster
• We have newly elected and/or appointed officers
• We are having an informal recruitment event and I need to see if the potential new members meet scholarship requirements
• I'm in a fraternity and we are having a party.
• I'm in a fraternity and we are having a brotherhood party? (A brotherhood party is a party with brothers only, brothers and dates only).
• I'm in a sorority and we are having a formal event or date/crush party
• I'm in charge of planning a philanthropy or community service event and we haven't registered it yet
• I'm in charge of planning a philanthropy or community service event and I need to report it's success
• My organization is having a mixer with another organization
• My organization is having an activity off campus without alcohol present
• My organization is participating in a fundraising activity
• My organization is having Initiation ceremonies

Remember, Greek organizations must register ALL EVENTS/FUNCTIONs with the exception of chapter meetings!
