Prior to Registration:

 All students must fill out Hepatitis B/Meningitis wavier form prior to registering for the initial semester.

Full-Time Registration Requirement:

Students must provide proof of two (2) MMRs (measles, mumps, and rubella) vaccinations.  Proof of immunity to measles, mumps, and rubella may be provided by meeting one of the following 3 criteria:

Student must also provide proof of (2) Varicella (chicken pox) vaccinations.  Proof of immunity to chickenpox is required by meeting one of the following 4 criteria:

All new incoming students living in on campus housing under the age of 22 must submit proof of adequate immunization before you will be allowed to live in on-campus housing if you are under 22 years of age. Adequate proof consists of documentation from a health care provider showing a single dose of conjugate vaccine (MCV4) given on or after the 16th birthday. If your initial vaccine for meningitis was giver prior to the age of 16 years, immunization requirements will not be met unless an additional boost vaccine is given after your 16th birthday. Booster doses are recommended for high risk patients every five years. College freshmen living in on-campus housing are at increased risk for infection.
