  1. PC 191 (Fee Waiver):
    1. Complete the PC 191 form pdf_icon_sm. Updated 11/26/07
    2. Obtain signatures of faculty advisor, supervisor, and academic/administrative officer.
    3. Submit completed form to Human Resource Services, Derryberry Hall, Room 146 or PO Box 5132 at least two weeks prior to registration.
  2. Employee Audit/Non-Credit Program
    1. Complete application form pdf_icon_sm and forward to Human Resource Services, Derryberry Hall, Room 146 or PO Box 5132, at least two weeks prior to registration. Updated 11/26/07
  3. Staff Scholarship (Clerical and Support Staff Maintenance Fee Payment Program)
    1. Complete an application form pdf_icon_sm (NCR paper required). Updated 11/26/07
    2. Obtain signatures of supervisor, departmental chair or director, and academic dean or administrative officer.
    3. If any courses are during the normal working hours, include an adjusted work schedule.
    4. Submit all copies of the completed form to Human Resource Services, Derryberry Hall, Room 146 or PO Box 5132 at least two weeks prior to registration.
  4. Faculty/Administrative Staff Tuition Reimbursement Program
    1. Complete application/contract form pdf_icon_sm. Updated 11/26/07
    2. Obtain signatures of departmental chair or director, and academic dean or administrative officer.
    3. Submit completed form to the President's Office, Derryberry Hall, Room 206 or PO Box 5007, at least two weeks prior to registration.
  5. Student Fee Discount for Spouse and Dependent Children of TTU Employees
    1. Complete fee discount form pdf_icon_sm Updated 12/06
    2. Submit completed form to Human Resource Services for certification of employment.
  6. Fee Discount for Dependents of Licensed Public School Teachers or State Employees
    1. State Employees:
      1. Complete the application form pdf_icon_sm.
      2. Attach a copy of your State ID.
      3. Obtain appropriate signature from your Human Resources Office. (Be sure to get employee and student signatures.)
      4. Return completed form to Office of Student Financial Aid, PO Box 5076, Cookeville, TN 38505 no later than two weeks prior to registration.
    2. Public School Teacher:
      1. Complete the application form pdf_icon_sm (Be sure to include your current license number).
      2. Obtain appropriate signature from your Human Resources Office. (Be sure to get employee and student signatures.)
      3. Return completed form to Office of Student Financial Aid, PO Box 5076, Cookeville, TN 38505 no later than two weeks prior to registration.
