Health Insurance

Health and accident insurance must be purchased when you register for classes. The plan provides hospital, surgical, basic medical, supplementary medical, and catastrophic services as well as evacuation and repatriation.

Details concerning this insurance are available at the time of registration. Insurance is also available for dependents if purchased within 30 days of their arrival in the U.S. Please refer to the Student Accident and Sickness Insurance Plan pamphlet available in the ISA office for more information or go to www.gmsouthwest.com.

Students may elect to purchase their own coverage but all personal plans must include: Medical Evacuation and Repatriation coverage.

Preferred Providers in your local school area are Humana/Choice Care Hospitals and Physicians. The availability of specific providers is subject to change without notice. You should always confirm that a Preferred Provider is participating at the time services are required by:

INSURANCE OPTIONS

IMPORTANT NOTICE: You may only choose the ANNUAL or Fall only /Spring only and/or Spring plus Summer or Summer only options in the FALL Semester enrollment period.

• Unless you elect to pay for a different premium, your student account will automatically be charged at Fall and Spring registration. You will be charged for Fall semester only ($298) when you register for the Fall semester; you will be charged for Spring and Summer ($489) when you register for the Spring semester. If you plan to graduate in the Spring you need to verify this with a letter from your academic advisor. This will provide you with insurance coverage for a full 12 months.

• If you elect to pay for insurance coverage at the annual, Fall or Spring semester rates please e-mail Brenda Gentry the insurance enrollment manager, at bgentry@tntech.edu for TTU International Student Affairs Office with your name, Social Security number and plan option, or you may complete the insurance form available in the Office of International Student Affairs.

NEW INSURANCE POLICY FOR THE 2006-2007 SCHOOL YEAR

Student over 40 years old may be required to pay more for their insurance. Please inquire with the Office of International Student Affairs. Policies for a year start at $816 for a student over 40 years old. A spouse's insurance fee who is over 40 years old will be charged $1968.
