ADDRESS CHANGE FORM for F-1 and J-1 students

* Federal SEVIS regulations require F and J students, and scholars (including students who have graduated but are on optional practical or academic training) to notify the Office of International Student Affairs within 10 days of any change in your U.S. address.

By reporting this change on this form, it will then be updated into SEVIS and you do not have to file a separate AR-11 form with the Department of Homeland Security.
