• Drop/Add Card
  May be obtained in the Student Success Center (Advisor's signature is required and must be returned to the Registration Help Window, Derryberry Hall, Room 162).
• Change of Major
  May be obtained in the Student Success Center (Advisor's signature is required and must be returned to the Records Office, Derryberry Hall, Room 102).
• Readmission After Suspension (Advisor's signature required)
• Request for Extension or In-Residence Study at Another Institution
  May be obtained from http://www.tntech.edu/records/pdf/extension.pdf (Advisor approval must be granted before enrolling in the extension course)
• Repeat Card (no signature required)
  May be obtained from the Student Success Center or outside of the Records Office in the forms rack.
