New Student Forms
New student forms are for incoming freshmen or transfer students who will begin classes at TTU Fall 2012 or Spring 2013.
With the exception of the Financial Aid Estimate Form and Letter of Recommendation Forms, all forms on this page should be submitted via the online scholarship application in ScholarWeb. The forms below are provided so you can preview the questions that will be asked of you before you begin the online application. If you are unable to apply online, you may print and submit the paper forms. Keep in mind that applying by paper will require additional processing time.
New Student Scholarship Application
To be considered for scholarships for 2012-2013, submit this application to the Scholarship Office, PO Box 5166, Cookeville, TN, 38505 by the scholarship application deadline, December 15, 2011. This one application will make you eligible for all scholarships for which you meet the criteria.
Letter of Recommendation Form (PDF)
Some departments require letters of recommendation for scholarship consideration. Review the Checklists by Major to see if your department requires this form.
Music Department Letter of Recommendation Form (PDF)
This form should be submitted directly to the Music Department, PO Box 5045, Cookeville, TN, 38505.
Financial Aid Estimate Form
Submit this form if you wish to be considered for need-based scholarships. Complete this form online using your 2010 tax information. Select Summary Output and Federal Methodology. After completing the form, click Calculate, print the form, and mail the results to the Scholarship Office. Be sure to include your name and T Number on the form.
The following departments require forms in addition to the Scholarship Application. Majors in the following areas, please also complete the appropriate departmental page:

- Chemistry/Health Sciences (PDF) (Includes Pre-Professional majors, but Nursing majors should not complete this form)
- Computer Science (PDF) (Mary Patterson Scholarship only; Computer Science majors should also complete Engineering Department Page)

Honors Academic Scholarship Application (PDF)
The Honors Program requires out of state students who wish to apply for the Honors Academic Scholarship to complete this application in addition to the New or Currently Enrolled Student Scholarship Application. For more information on the Honors Program and Honors Academic Scholarship, visit the Honors Program Website. Please note the HAS is for out of state students only.
Currently Enrolled Student Forms
Currently enrolled student forms are for students currently enrolled in classes at TTU or who will be enrolled prior to the Fall 2012 term. This includes students beginning classes Spring 2012.

Statement of Goals (PDF)
All students must submit a Statement of Goals to be considered for scholarships except those majoring in Agriculture, Chemistry/Health Sciences, Computer Science, History, Human Ecology, Math, and Special Education.
All departmental forms and letters of recommendation must be submitted to the Scholarship Office, PO Box 5166, Cookeville, TN, 38505 by January 15. Be sure to include your full name and T Number on every item that you submit.
