Call the UC-CCR&R at 931-372-3780 or 1-888-621-5753 (Toll Free), and provide the following information:

bullet Your name and the name of your facility
bulletPhone number where you can be reached in case of cancellation
bulletNumber of people coming with you
 

Please mark your calendar as a reminder for yourself and your staff. If you are unable to attend the workshop you registered for, please call us to cancel.

It is important that you register for any TN-CCPT Workshop you wish to attend.
