Concerned about your child's development?

Milestones serves families with children birth to age three (3) throughout Middle Tennessee, as an independent evaluation agency.

As Tennessee’s Early Intervention System (TEIS) receives referrals from agencies or individuals with concerns about a child’s development, a Milestones evaluator schedules a visit with the family to conduct a developmental evaluation.

The results of the evaluation help to determine if the referred child is eligible for early intervention services at no-cost to the family under the TEIS service provision outlined in Part C of the Individuals with Disabilities Education Act (IDEA). Evaluators also attend initial Individualized Family Service Plan (IFSP) meetings, when possible, to explain evaluation results for children determined eligible.

If you have concerns about your child’s development and would like to refer your child for an evaluation to determine eligibility for early intervention, please complete the referral form below.

TEIS online referral form

OR by calling 1-800-852-7157;
OR accessing the referral form in the MyTN app, available from the app store.

The current eligibility evaluation tool chosen by Tennessee is the Battelle Developmental Inventory, 2nd Edition, Normative Update.

Milestones Eligibility Evaluation Grant

The Milestones Eligibility Evaluation grant is a partnership with TEIS providing services to families eligible for early intervention services as outlined in Part C of the Individuals with Disabilities Education Act (IDEA).
