If you are enrolled in the Partnership PPO health insurance plan, the deadline to complete the online Well-Being Assessment (WBA) is midnight Friday, March 15.
If you (or your covered spouse) have trouble accessing the Well-Being Assessment, call 1-888-741-3390 and press 2 or email Healthways for assistance at: email@example.com.
Healthways is experiencing a very high call and email volume. However, the longest you will have to wait on hold is five minutes before you are directed to leave a message. If you have trouble getting through on the phone, leave a message with your name, member ID (found on your Caremark card) and phone number. Someone will call you back within 10 business days. Include the same information if you email Healthways.
If you do this, Healthways will follow up with you or your covered spouse and, even if it is after March 15 when you hear from them, you will get credit for meeting the deadline if you complete the Well-Being Assessment.
Again, if you have called this week and left a message or sent an email to Healthways regarding login information or because you have been unable to complete the Well-Being Assessment online, they will have a record of that call or email and you will not be penalized for being unable to meet the March 15th deadline.
Healthways will extend their call center hours until midnight on Thursday and Friday. Their usual hours of operation are:
- Monday – Friday, 7:30 a.m. to 9:30 p.m. (CT)
- Saturday from 8 a.m. to 6:30 p.m. (CT)
If you are only calling or emailing to check the status of your WBA, you can do so online in Well-Being Connect. Click Here <https://partnersforhealthtn.embrace.healthways.com/healthways/login>; . When you log in, first make sure you have completed your Well-Being Plan. It only takes a couple of minutes. Then click on the Rewards Center tab on the top right to check your status. If you see two green stars next to WBA, you have completed it.
